Excited for this one actually. Asthma's one of those things that, well, you see it a lot, but people don't always know what to look for, do they?No, not at all. And I think a lot of people just think of wheezing, but there’s so much more to it. Like, you’ve got coughing, chest tightness, sometimes just a bit of shortness of breath, especially after exercise or at night. It’s not always dramatic, is it?Exactly. And sometimes it’s just that tiredness, you know? People don’t realise that fatigue can be a sign, because you’re not getting enough oxygen. But then, when it gets severe, that’s when you see things like really rapid breathing, or someone struggling to get their words out. I remember this one time, actually, when I was volunteering at a local football match—Oh, go on, I love a good story.Yeah, so there was this lad, probably about twelve, and he started coughing a bit, nothing major at first. But then he just went really quiet, and I noticed he was using his neck muscles to breathe, like you could see the effort. His lips started to go a bit bluish, and he couldn’t finish a sentence. That was the moment I knew it was serious. We got his inhaler, but honestly, it was that quick recognition that made the difference. If we’d waited, it could’ve been a lot worse.That’s so important, isn’t it? Spotting when it’s gone from just a bit of wheezing to something much more dangerous. I always say, if someone can’t speak in full sentences or their lips are turning blue, that’s a big red flag.Yeah, and sometimes people think, oh, they’ll be fine, just give it a minute. But with asthma, you really can’t take that risk.So, let’s talk about what you actually do if someone’s having an asthma attack. First thing, get them sitting upright, not lying down. That helps open up the airways a bit, doesn’t it?Yeah, definitely. And try to keep them calm, which is easier said than done, but it really does help slow the breathing. Then, you want to get their reliever inhaler—usually the blue one, like salbutamol or ventolin. If they’ve got a spacer, use that, because it gets more of the medicine into their lungs.And it’s not just one puff and done, is it? You give one puff every thirty to sixty seconds, up to ten puffs. I always have to double check that, because it sounds like a lot, but that’s what the guidance says.Yeah, and you keep an eye on them the whole time. If there’s no improvement after four minutes, or if they’re getting worse, that’s when you need to think about calling for help. But before we get to that, Tina, didn’t you have a situation at work once?Oh, I did, yeah. It was a few years back, actually, in the IT office. One of my colleagues started coughing and looked really uncomfortable. At first, I thought it was just a bit of dust or something, but then she said her chest felt tight and she was struggling to breathe. Luckily, she had her inhaler in her bag, but she was so panicked she couldn’t get it out. I helped her sit up, got her inhaler, and used her spacer. We did the puffs, and after a couple of minutes, she started to settle. It was scary, but just following those steps made all the difference.That’s the thing, isn’t it? It’s about staying calm, helping them with their medication, and not leaving them alone. And don’t give them food or drink, even if they ask for it. That’s something people forget.Yeah, and don’t assume it’ll just go away on its own. Asthma attacks need action, not waiting around.So, when do you actually call for emergency help? I think this is where people get a bit stuck, because they don’t want to overreact. But if someone’s not improving after their inhaler, or they’re getting more distressed, that’s the time to call 999 or your local emergency number. If they’re drowsy, confused, or can’t speak, don’t wait.Absolutely. And while you’re waiting for help, you can keep giving one puff every thirty to sixty seconds, up to another ten puffs, every fifteen to twenty minutes if you need to. But you stay with them, keep reassuring them, and don’t leave them alone. That’s so important.Yeah, and don’t give them food or drink, like we said before. I know it’s tempting, especially if they’re panicking, but it’s not safe. And don’t just assume it’ll sort itself out. I remember reading an NHS case study where someone delayed calling for help, and it made things so much worse. The earlier you call, the better the outcome, honestly.And sometimes, even if you’re not sure, it’s better to be safe. Like we talked about in the diabetes episode, it’s always better to act than to hesitate. You can’t go wrong by getting help if you’re worried.Exactly. And just to wrap up, remember: spot the signs, act quickly, and don’t be afraid to call for help. That’s what makes the difference in an asthma emergency.Brilliant.
